The National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care was introduced in 2007. It was revised in 2009 as a result of a commitment to review the National Framework within 12 months of its publications and revised again in 2012 to reflect the new NHS framework and structures created by the Health and Social Act 2012, effective from 1 April 2013. The associated tools; checklist, decision support tool and fast track pathway tool which are designed to assist clinicians and practitioners with the decision making process have also been revised accordingly.
